PATIENT PARTICIPATION PLAN FORM

How to Play HMO HARDBALL teaches patients all about dirty tricks that HMOs and their doctors use to keep patients in line.  For example, Dirty Trick #27 ("Resist When They Insist") creates stress and tension for patients, so that they stop insisting on equal power with their doctors in medical decision-making.

HMO doctors used this trick on two of my friends who inspired me to create this form.  It was too late for one.  He died.  It's not too late for my other friend.  She'll either get what she needs or can make her HMO and its doctors feel her pain if they won't treat her as an equal.

Don't be an HMO sob story.  Participate in medical decisions as an equal.
